Parent authorisation form

Dear parents and guardians,

Every five years, people attending churches across Australia take part in the National Church Life Survey. The aims of the survey are to carry out research to assist churches in their ministry and mission and to provide statistics about churches in a changing and diverse Australia.

This year, a special survey will be offered to children aged 8 – 14 who attend church’s activities. This survey, which is completely anonymous, will be made available during a church’s normal activities. There is no obligation on your child or teenager to complete this survey.

An authorisation form is available below, to make parents and guardians aware of the purpose of the survey and giving you the option to decline participation. If you do not wish your child or teenager to take part in this survey please return this note to your church. Otherwise we will assume that you see no problem in your child taking part.

About NCLS Research

The National Church Life Survey is a five-yearly co-operative venture across all churches in Australia, carried out by NCLS Research. Around 400,000 attenders in 7000 churches in 22 Christian denominations have taken part in previous National Church Life Surveys in 1991, 1996, 2001 and 2006.

The protocol of the 2011 National Church Life Survey has been reviewed and approved by the Australian Catholic University (Human Research Ethics Committee).

NCLS Research is jointly sponsored by ANGLICARE Diocese of Sydney, the Uniting Church Board of Mission (Synod of NSW & the ACT), the Australian Catholic Bishops Conference and the Australian Catholic University.
